Please be on time for your WIC appointment. If you are late, your appointment may need to be rescheduled. Please do not bring sick children. If your children are ill, please re-schedule.
What to bring to your appointment: |
| •All individuals applying for WIC |
• WIC Packet (if you have one) |
| •Proof of Identification |
• Proof of Income (Pay stubs) |
| •Proof of residency |
• Immunization record for each child |

How to use your WIC Vouchers 
- As you shop, separate WIC food from other food items
- When you arrive at the check-out, tell the cashier you have a WIC purchase
- Give the cashier the WIC vouchers you will be using
- Show the cashier the your WIC Packet.

| You can use your WIC Vouchers at participating local stores in Utah. If you are unsure whether a store is a WIC Retailer, ask the store manager or the WIC Clinic Staff. |
