The Tooele County School District
works in conjunction with the Tooele County Health Dept. to provide
various health screenings throughout the school year in order to promote
good health in our schools. It is our intent to provide the parent
with this information, and to obtain a written consent for these services.
The following link will take you to the consent form. Print it on
your printer, and check those items that we have permission from you
to provide to your student. If we do not have your consent, please
mark the box labeled "No". Then, sign and return this form
promptly to your child's school. Thank you.
