Utah's new Health Insurance
Program for Working Adults
 

The Utah Department of Health's Primary Care Network (PCN) is designed to provide health insurance coverage for working individuals who are unable to afford a private health plan.

Eligibility: Those who are eligible:

Health Insurance for AdultsAdults 19 to 64 years who have not had health care coverage for at least six months and do not qualify for Medicaid

Health Insurance for AdultsAdults who are residents of Utah

Health Insurance for AdultsAdults who are U.S. Citizens or have certain types of permanent residence status

Health Insurance for AdultsAdults whose annual income is within the PCN guidelines (see table below)

  Qualifications: PCN qualifications are based on income alone. The income guidelines are:

Using the online application is the easiest and quickest way to apply. You can also apply at the Workforce Service Building at 305 North Main Street, Tooele.

If you need help completing the application or would like to get additional information about the PCN program, please contact the Health Resource Hotline at 1-888-222-2542 to have an application mailed to you.

Another service available is the Rx Connect Utah Program. This program is offered from various drug manufactures and may be able to help provide free or reduced medications. Utah pharmacies have tear off pads which can be given to patients who come in to have prescriptions filled and find out they don't have insuranceLink to RxConnect Utah coverage to help pay for these medications. The tear off sheet gives the phone number and web site for this new program.
