Once you have become approved for Medicaid through the Department of  Workforce Services (located at 305 North Main Street in Tooele), you will need to choose your Primary Care Provider. The Medicaid Health Representative at the Tooele County Health Department will help you with this process. Please phone (435) 277-2309 and ask to speak with the Medicaid Health Representative.  If you wish to make a change once you choose your Primary Health Care Provider, you must do it by the 20th of the month to have it changed for the following month.  If you are not eligible for Medicaid, you may qualify for Utah's new Adult Insurance Program for Adults(PCN).
 

Utah residents may also quality for RxConnect Utah. This program is offered from various drug manufactures and may be able to help provide free or reduced medications. Utah pharmacies have tear off pads which can be given to patients who come in to have prescriptions filled and find out they don't have insuranceLink to RxConnect Utah  coverage to help pay for these medications.
