Pick up your TooeleCountyRx Prescription Drug Discount Card Information Sheet at one of the local pharmacies in Tooele. You will also find them in government offices, the public library, and other agencies. You may also pick one up at the health department, 151 North Main in Tooele.
After you pick up your card, have your pharmacist activate it the first time you use it. You mayt activate it yourself by calling 1 - 877 - 321 - 2652. |
