for all babies and children up to the age of 5.

277-2310 to schedule a home visit with your nurse. She will come to

See how you and your newborn or child are doing
Assess any of your needs or concerns
Refer you to other programs, as needed
Assist you with finding a primary care provider, or other resources
Provide you with support and guidance
Some families may only need one visit. Other families may need
several. Home visits will stop when your family no longer needs
them.
A Home Visit is convenient and you get personal one-on-one attention
